This is a used Mayware Formula IV Stereo Tonearm. It includes the main counterweight and arm rest as shown. This tonearm is in need of some work on the wiring as the plug was pulled on the headshell end (see pictures). Also, the small bias weight is missing. Otherwise, this is a nice tonearm for your turntable.
