Canada Revenues EZ, Unused WW2 Ration Form that asks for Coal.  The inscription "M. & S. 4172 (Rev.) / K.P. 19975" in top left corner on back is the form's identifier, where 'M. & S.' stands for "Munitions and Supply" and '41' possibly identifies that the form's production year was 1941.  This is a nice historical document.
